Two Clinical Benchmarks Come First: BMI and Waist Circumference

Korean clinical practice classifies obesity using two reference points. The Korean Society for the Study of Obesity's 2022 guideline classifies adults with a BMI of 23 kg/m² or higher as pre-obese and 25 kg/m² or higher as obese, and defines abdominal obesity as a waist circumference of 90 cm or more in men and 85 cm or more in women. The same guideline raises the recommended intensity of management when abdominal obesity is present alongside a high BMI.

BMI is simple to calculate but blind to composition. The Korean Society for the Study of Obesity notes that someone with high muscle mass can be classified as obese without high body fat, and that BMI in the 18.5 to 24.9 kg/m² range does not reliably reflect actual fat mass. Waist circumference has its own measurement sensitivity: it is meant to be taken midway between the lowest rib and the top of the pelvic bone, without the tape compressing soft tissue.

A body composition scan is meant to fill in what those two numbers leave out. A 2016 research report from National Health Insurance Ilsan Hospital found that combining BMI with body composition analysis identified high-risk groups for obesity-related conditions more effectively than BMI alone, and helped flag cases of normal-weight obesity that BMI would otherwise miss.

How a Body Composition Analyzer Reads the Body

Devices in the InBody family rely on bioelectrical impedance analysis. Muscle carries a relatively high water content, so electrical current passes through it with less resistance than through fat, and the device converts that resistance into estimates for body fat mass, skeletal muscle mass, visceral fat area, protein, and minerals. The same Ilsan Hospital report describes the test as radiation-free and completed within minutes.

That convenience comes from a formula, and the formula is where the limits begin. A systematic review comparing BIA devices against four-compartment reference models examined 12 studies in healthy adults and found that BIA devices did not consistently agree with reference-model estimates of body fat percentage and fat-free mass. Variation between the included studies was large enough that the reviewers did not attempt a pooled meta-analysis.

More precise fat measurement exists in the form of DXA scanning, which the Korean Society for the Study of Obesity acknowledges can measure body fat percentage accurately, though the equipment is expensive and requires trained staff to operate. Bioelectrical impedance testing remains the everyday clinical choice not because it is the most accurate method available, but because it avoids radiation and can be repeated quickly at low cost.

Why the Same Body Can Produce Different Numbers

A single reading is a snapshot influenced by what you ate, drank, and did in the hours before the test. Repeat measurements only become meaningful when the conditions around them are consistent.

A 2025 study in the European Journal of Clinical Nutrition compared the InBody 770 against DXA in 1,000 active-duty US Marines under real-world conditions, without controlling meals, hydration, or exercise beforehand. The study reported high test-retest reliability and moderate agreement with DXA for body composition estimates other than visceral fat. Because the study population was healthy young military personnel, its findings do not automatically extend to every patient group.

That is why we treat pre-test guidance as part of the consultation rather than a formality. Testing at a similar time of day, under similar meal and hydration conditions, and away from strenuous exercise makes the trend line across visits more informative than any single value.

We do not treat body fat percentage as a standalone pass-or-fail figure, and neither does the field guideline. Body composition data works best as a layer added to that judgment, not a replacement for it.

Setting a Weight-Loss Target You Can Actually Sustain

The same Korean guideline offers a starting number for goal-setting: a 5 to 10 percent reduction from baseline weight within six months as the initial target, followed by long-term management for a year or more to help prevent and manage obesity-related conditions.

That range can look modest next to more aggressive claims elsewhere. Read alongside its own recommendation for sustained follow-up, though, a target you can hold onto for a year is easier to build a consultation plan around than one aimed only at the fastest possible drop.

Protecting Muscle While the Number on the Scale Moves

Weight loss rarely subtracts fat alone. A 2025 narrative review on weight loss in the era of incretin-based medications notes that while these medications can produce substantial weight loss, they can also accompany a rapid decline in muscle mass and bone density, and that adequate protein and fiber intake combined with structured resistance training play an important role in limiting lean mass loss and preserving functional capacity.

Combining diet and exercise shows measurable results in controlled trials as well. A 10-week randomized controlled trial in overweight and obese adults aged 55 and older in Amsterdam evaluated how a high-protein diet paired with resistance training affected lean mass preservation, fat mass loss, waist circumference, grip strength, and physical performance during a weight-loss period. Because the trial was limited to a specific age group and set of conditions, the exact diet and exercise intensity for any individual needs to be set in consultation rather than copied from a study.

This is the point where body composition data earns its place again. If two kilograms come off the scale, knowing whether that loss came from fat or muscle changes what the next month's plan should look like.
